Requirements & Fees
- Cash, Check, or Credit Card accepted
- Valid identification is required for all requests
- Birth Certificates: $25.00 for the first copy, $20.00 for additional copies
- Death Certificates: $25.00 for the first copy, $20.00 for additional copies
Birth Certificates
Birth certificates may be obtained locally if the individual was born in Colorado.

New for 2026: Colorado 150 Year Logo on birth certificates
- As part of the celebrations for the 250th anniversary of the Declaration of Independence and the 150th anniversary of Colorado’s statehood, the Colorado 150 logo is available on certified copies of birth certificates upon request.
- There is no additional charge to the customer to purchase a birth certificate with the logo.
- To be used only for certificates issued between 1/1/2026 and 12/31/2026.
Death Certificates
Death certificates may be obtained locally if the individual died in Colorado.

You can request a birth or death certificate to Gunnison County Department of Health and Human Services. Alternatively, you can email your request form and supporting documents to hhsadminteam@gunnisoncounty.org with the subject line Attn: Vital Records, or mail the form to the local office at: Attn: Vital Records 220 N. Spruce St. Gunnison, CO 81230.
